Eligibility and Process

We accept requests from hospitals, healthcare facilities and providers, physicians and family. Each request will be reviewed. We ask that you complete the Contact Form with as much information as possible and submit. If you contact us by phone, the same information will be taken and a healthcare provider will return the call. Due to privacy issues, a Release of Information form will be sent by email/FAX for the patient or POA to sign.

Since, we assist with medical transport by air ambulance, commercial airline and ground transport, costs will be assessed by the Global Angel Charitable Organization representative after determining the safest method of transport.
*Medical condition, urgency and destination will determine the method of transport and costs involved, if any.
The information will be discussed with family, and healthcare providers (if applicable). Once all parties have reviewed the plan, an agreement form will be sent for the legally responsible person to sign allowing us to proceed with the medical flight or ground service.
PLEASE NOTE: The time frame for a flight/ground service varies from same day to months later depending on the urgency and long term planning by the family or healthcare facility.
